If you can help drive for the teams or are a student driving to practices,

please fill out these VUSD Driving Forms

and return to the Athletic Office with a copy of your

CA Driver's License and Insurance Card:

Need a Physical?

Here are two options if you need a sports physical quickly. Please make sure you call and make an appointment (usually same or next day appointments are available).

Be sure to bring the necessary VUSD Physical forms to the clinic for proper signatures.

Spring Medical Clinic


1671 E. Monte Vista Ave. #213

Hours: Tuesday (9-5pm), Wednesday (8-4pm), Thursday (9-5pm), Friday (9-5pm) 9:30-5:30pm

and Saturdays (8-12:00pm).

Cost: $55.00 (cash, credit or debit only)
